• Plan and conduct intervention opportunity evaluations, respond to urgent alerts and remote patient monitoring alerts as needed to help drive high quality care at a lower cost
• Have the ability and skill to recognize clinical scenarios that require escalation to the internal team nurse practitioner
• Work directly with the member, via various forms of communication, texting, virtual visits, and telephone, to develop and achieve patient centered chronic care management goals
• Develop and update care plans for members while keeping a close eye on caregiver and/or family support
• Apply clinical experience and judgment to the utilization management/care management activities
• Be responsible for day to day work with patients related to interventions needed for quality outcomes to reduce avoidable admissions, readmissions and ED utilization.
• Collaborate with engagement and product teams to promote quality outcomes, optimize service experience, and promote effective use of resources for complex or elevated medical issues

Would you describe yourself as someone who has:

• Can commit to a full-time opportunity working weekends and some weekdays (required)
• Is fluent in English AND Spanish/Russian/Mandarin (required)
• Has a compact nursing license (required)
• Graduated from an accredited nursing program (required)
• Current RN License in good standing in the state of New York AND a Compact License (required)
• At least 2 years of nursing experience providing care to adult and geriatric patient populations (required)
• Confidence with clinical skills in performance of telephonic triage/assessment (required)
• The ability to work remotely and has a private area in their home/workspace (required)

• Plan and conduct intervention opportunity evaluations, respond to urgent alerts and remote patient monitoring alerts as needed to help drive high quality care at a lower cost
• Have the ability and skill to recognize clinical scenarios that require escalation to the internal team nurse practitioner
• Work directly with the member, via various forms of communication, texting, virtual visits, and telephone, to develop and achieve patient centered chronic care management goals
• Develop and update care plans for members while keeping a close eye on caregiver and/or family support
• Apply clinical experience and judgment to the utilization management/care management activities
• Be responsible for day to day work with patients related to interventions needed for quality outcomes to reduce avoidable admissions, readmissions and ED utilization.
• Collaborate with engagement and product teams to promote quality outcomes, optimize service experience, and promote effective use of resources for complex or elevated medical issues

Would you describe yourself as someone who has:

• Fluency in English AND Spanish OR Russian in writing, reading, and speaking (required)
• Graduated from an accredited nursing program (required)
• Current RN License in good standing in the state of Massachusetts (required)and a New York license (preferred)
• At least 2 years of nursing experience providing care to adult and geriatric patient populations (required) 
• Confidence with clinical skills and knowledge of chronic conditions (required)

A customer focused individual who is responsible for assisting the team in coordinating the care of members in our Triage department. This will primarily entail telephonic outreach calls to members, caregivers, and other care team members as directed with documentation in the appropriate platform to ensure compliance. The Triage Care Coordinator will collaborate with the supervising provider and staff to conduct outreach, assessment and service planning to coordinate care for the Triage department.

The ideal teammate would be able to:

• Provide practice support including: contacting members, caregivers, and care team members as directed, work closely with the clinical team to improve the health and care of our members. 
• Support patients and their caregivers by serving as their advocate and resource.This includes (but is not limited to): making follow up phone calls to members, assisting with remote patient monitoring (RPM), and performing care coordination. 
• Navigate various platforms for documentation and retrieval of member information.
• Data entry within operating dashboards, reporting and workflow platforms 
• Ensure Data Quality and Accuracy
• Other administrative support

Would you describe yourself as someone who has:

• Certified Medical Assistant (required) 
• Bilingual with fluency in English AND Spanish, Cantonese, Russia OR Mandarin (writing, reading and speaking) (required)
• 2+ years of experience in an urgent care, internal medicine clinic, family medicine clinic OR cardiology clinic  (required)
• Experience communicating with patients via telephone and/or telemedicine (required)
• Experience working in a fast paced environment with the ability to multitask (required)
• The ability to work flexible hours with rotating weekends (required)

• Perform comprehensive medication management, chronic disease state management, and/or medication reconciliation via virtual patient visits
• Identify how patients are taking all of their medications, including OTCs, herbals, DME and provide solutions for adherence/administration, inappropriate therapy, etc.
• Support medication education/counseling to help improve patients’ understanding of medications
• Evaluate each medication for appropriateness, effectiveness, interactions, and safety
• Identify patients who have not achieved clinical goals of therapy and provide appropriate recommendations to the care team and/or patient/caretaker
• Identify opportunities to close quality gap measures
• Identify, manage, and document drug therapy interventions 
• Consult with patients’ care team to ensure clinical recommendations are considered
• Serve as a medication expert and resource for the clinical care team

Would you describe yourself as someone who has:

• Graduated from an ACPE accredited school of pharmacy in the United States with a Doctor of Pharmacy degree (required)
• A current, unrestricted pharmacist license in good standing (required)
• A valid NPI number (required)
• 2+ years of direct patient care or ambulatory care experience performing comprehensive medication management where you were directly involved or part of an interdisciplinary care team to manage patients’ chronic conditions (excluding clinical rotations in pharmacy school) (required)
• Bilingual in English and Spanish (writing, reading and speaking) (required)
• Been actively seeing patients in their current or most recent role (required)
• The ability to practice independently with little clinical support (required)

The Medical Assistant will collaborate with the supervising provider and staff to conduct outreach, assessment and service planning to coordinate care for the CCM patients.
